The problem

A hospital is several businesses running at once: a registration desk, consulting rooms, wards, a pharmacy, a laboratory and an accounts office. Each of them keeps its own register, and the patient's file is whatever paper can be found when it is needed.

So a discharge takes an afternoon of collecting chits from four departments, a result goes missing between the laboratory and the ward, and at the end of the month nobody can say what a department earned. Shifa keeps one file per patient from registration to discharge, and every charge raised anywhere in the hospital lands on the same bill.
